Making a complaint about your NDIS provider

If the Participant or Guardian/Authorised Representative wishes to give the Provider feedback, complaints or suggestions to assist with continuous improvement, we encourage contact via;

If the Participant is not satisfied with the response from Mawarnkarra Health Service including the opportunity to escalate your concerns through Mawarnkarra Health Service management, the Participant can contact the NDIS Commission Complaint.

How to complain to the NDIS Commission

Anyone can make a complaint to the NDIS Commission about a NDIS service provider. This includes an NDIS participant, their family members/friends, their NDIA appointed nominee, advocates, guardians or any other person who wishes to make a complaint.
