POSITION OBJECTIVE
To provide a culturally appropriate service, including assisting with accessing holistic health care services to clients.
The role of the Sexual Health Outreach Worker is to ensure that the program objectives are met whilst working under the supervision and direction of the Clinical Manager. The focus will be to meet the needs of the target community by providing health services, at times out of hours, to the targeted population. This will be achieved by working in a team environment, coordinating and collaborating with other team members and supervising staff to do all things to plan and deliver services and monitor and evaluate outcomes.

DUTIES
1. Community member responsibilities
☑ Community liaison: Establish strong links in the broader Aboriginal community for the purpose of establishing connections and community referrals to services.
☑ To establish links within the community, encourage and support target population to undertake health services, including: health checks and medical services when required.
☑ Provide education and awareness raising programs within the community about accessing health services for sexual health.
☑ Practical assistance: To provide practical assistance to individuals within the community by assisting with patient transport services if needed, to ensure the individuals can access health services.
☑ Provide support in ensuring an adherence to medication regimes, follow up appointments and attendance at appointments.
☑ Assist with transportation to individuals when required.
☑ Provide follow-up support to individuals.
☑ Administration and support: Provide input and feedback to the MHS team on any access issues or barriers that are deterring or limiting access to services.
☑ Identify and work with the team to formulate solutions to access and care barriers.
☑ Teamwork and program implementation and coordination: Work collaboratively within a multi-disciplinary team.
☑ Establish strong links with other service providers providing services to target population for the purpose of enhancing client services and coordination of services.
☑ Feedback and access: Provide input and feedback to the MHS team for the purpose of enhancing early identification of those who would benefit from health checks and extended health services.
☑ Provide input and feedback on how to address barriers for the target population in accessing services.
2. Program responsibilities
☑ Work collaboratively to write a plan for the implementation of the program.
☑ Meet regularly with the Clinical Manager & Sexual Health Registered Nurse to report on progress and implementation of program.
☑ Develop a plan for data collection, review and feedback to the team and MHS.
☑ Collect data daily and report on data at least weekly.
☑ Assist other staff in the collection of relevant data.
☑ Educate other team members on the program and their role in implementation.
3. Resources
☑ Use of a pool vehicle during work hours and for work purposes (no personal use and no home garaging).
☑ Use of a mobile phone during work hours for work purposes (no personal use).
☑ Comply with policies and procedures for use of vehicles, phones and other resources.
4. Professional and other responsibilities
☑ Participate positively in the work of MHS.
☑ Undertake any task not included in this position description that is reasonably requested and required.
☑ Maintain the highest standards of professional conduct.
☑ Contribute to the achievement objectives of the service.
☑ Demonstrate an understanding of and compliance with all Mawarnkarra policies and procedures at all times.
☑ Participate in maintaining a safe working environment.
☑ Other duties as directed by managers.
EDUCATION
Attend and participate in the professional development program and relevant external seminars and conferences, as approved.
OTHER RELEVANT INFORMATION
Performance will be appraised on an on-going basis and formally at the first 3 months and annually thereafter.
Consent to undergo a Working With Children Check and a National Police Certificate. Employment is conditional pending the outcome of these checks.
Current Drivers Licence.
All employees are subject to random drug & alcohol testing.
