POSITION CONTEXT
The Social Worker is part of the Clinical Services Team of Mawarnkarra Health Service. The Service objective is to provide holistic, primary health care services to Aboriginal people of the West Pilbara region.

CONDITIONS OF EMPLOYMENT

All Mawarnkarra employees are required to comply with all policies in force at the time of commencement of employment and those implemented from time to time.
As part of the orientation program, employees are required to sign declarations and agreement pursuant to policies of the Health Service.
POSITION OBJECTIVE
The Social Worker functions autonomously and collaboratively within the multidisciplinary
team of MHS.
The Social Worker provides services focusing on 0-5 year old year but not limited to the age
group of children as well as expectant mothers’ within MHS that contributes to the social and emotional well-being of the client and family.
The Social Worker possess knowledge and understanding of primary health care principles,
a demonstrated high level of verbal and written communication skills.

DUTIES
- Clinical
- Plan, coordinate, deliver and evaluate a culturally appropriate Social Work Service focusing mainly on the age range but not limited to 0-5-year-old clients, parents/guardians as well as expectant mothers’ using group and community-based strategies.
- Address each case as a unit and set tailored measurable goals.
- Monitor and evaluate clients’ progress and modify treatment plans accordingly.
- Participate in case conferences.
- Offer information and counselling on the best course of action during sessions.
- Act as a key-worker and cooperate with multidisciplinary teams.
- Refer clients to community services as necessary.
- Work collaboratively to support medical, midwifery, nursing and allied health colleagues in the performance of clinical duties.
- Manage patients and documents with regard to risk management principles.
- Leadership and Management
- Promote effective clinical leadership.
- Develop good communication skills in order to build solid relationships with all stakeholders in the organization, clients and families.
- Develop and promote positive relations with clients, peers, medical staff, allied health colleagues and members of the general public.
- Participate in relevant committees and special projects.
- Contribute to staff and patient safety systems that meet contemporary clinical standards and OS&H requirements.
- Liaise with other agencies and organisations as required.
- Continuing Quality Improvement
- Participate in Clinical Practice Improvement Programs and Clinical Governance Initiatives as relevant to practice.
- Identify opportunities to improve and promote evidence based clinical practice
- Ensure nursing services are provided in a manner sensitive to, and appropriate for Indigenous peoples and communities

- Other duties as requested by management.
- Essential Criteria
- Tertiary qualification in social work and eligible for full membership of the Australian Association of Social Work or the Society of Professional Workers
- Demonstrated knowledge and skills in assessment, treatment and evaluation within social work practice
- Demonstrated ability in applying time management and organizational skills when planning, providing and monitoring Social Work services.
EDUCATION
Attend and participate in the professional development program and relevant external seminars and conferences, as approved.
OTHER RELEVANT INFORMATION
Performance will be appraised on an on-going basis and formally at the first 3 months and annually thereafter.
Current Drivers Licence.
Current CPR Certificate.
COVID-19 Vaccinated. (triple vax as per government mandate for all health care workers)
Consent to undergo a Working With Children Check and a National Police Certificate. Employment is conditional pending the outcome of these checks.
All employees are subject to regular and random drug and alcohol testing.
